โEach aircrash is Unique & reaching till the centre of the Cause during investigation always brings multi-dimensional challenges along.
โThe factors that influence the Aircrash investigation are :
๐ Human Factors Involved.
๐ Photography of the Process.
๐ Witnesses (Direct /indirect).
๐ Records (Operations, Maintenance, Safety, Administrative, Weather Reports).
๐ Material Factors.
๐ Evidence Gathering.
๐ Analysis (Regular & critical).
๐ Conclusions & Validation.
๐ Recommendations (to prevent future accidents).
๐ Reporting.
Investigator in Charge’s role is Vital :
โThe Investigator in Charge (IIC) must have the authority to conduct the investigation, acquire resources, & have access to an organizationโs leadership.
โThe IIC should be senior in authority to the person directly involved in the accident to eliminate influence over the reported outcome of the investigation.
โThe IIC should also ensure that team members are well trained, equipped, & that they have no conflicting duties that take precedence over the investigative effort.
